Home Care Services

Home Care Services in the Northern Health Region include the following:

• Case coordination and management through assessment and care planning;
• Palliative Care planning and services;
• Assessment for, and referral to, Palliative Care, Personal Care Home placement, Supportive Housing and Adult Day Programs;
• Assistance with personal care and activities of daily living;
• Nursing care (i.e. treatments, dressings);
• Footcare;
• Respite care to relieve your caregivers through in home respite or respite in a designated Personal Care Home;
• Occupational therapist;
• Support with the provision of equipment and medical supplies for the home;
• The provision of oxygen concentrators and professional follow-up for eligible individuals who require continuous oxygen through the Home Oxygen Program.
