Home Care Services

Home Care Services in the Northern Health Region include the following:

Case coordination and management through assessment and care planning
Palliative Care planning and services
Assessment for, and referral to, Palliative Care, Personal Care Home placement, Supportive Housing and Adult Day Programs
Assistance with personal care and activities of daily living
Nursing care (i.e. treatments, dressings)
Respite care to relieve your caregivers through in home respite or respite in a designated Personal Care Home
Occupational therapist
Support with the provision of equipment and medical supplies for the home
The provision of oxygen concentrators and professional follow-up for eligible individuals who require continuous oxygen through the Home Oxygen Program.
