Home Care
Home Care is a comprehensive community-based program that provides essential support in the home to anyone, regardless of age, who requires health care services or assistance with daily living activities in their home.
Referrals to Home Care can be made by a nurse, family member or doctor. Home care can be provided on a short-term basis, for example, to provide help following a hospital discharge. This is not an emergency service. Please plan ahead.

Eligibility For Home Care
The Northern Health Region Home Care program is a community program that provides home health services and support with activities of daily living. Individuals who are residents of Manitoba and are registered with Manitoba Health (MH) may be assessed to determine whether they are eligible for home care services. Home Care services can be requested by you (as an individual), your family or by your doctor through a home care referral or by contacting your closest Home Care program office. After the Home Care program receives a request for service, a Home Care Case Coordinator will meet you at home to discuss your care needs and eligibility and if home based services are right for you. The Home Care Case Coordinator will use this information to develop a care plan that guides your care. The goal is to develop a plan of care with you that will help you manage at home as safely as possible for as long as you can.
Home Care Services
Home Care Services in the Northern Health Region include the following:
- Case coordination and management through assessment and care planning
- Palliative Care planning and services
- Assessment for, and referral to, Palliative Care, Personal Care Home placement, Supportive Housing and Adult Day Programs
- Assistance with personal care and activities of daily living
- Nursing care (i.e. treatments, dressings)
- Respite care to relieve your caregivers through in home respite or respite in a designated Personal Care Home
- Occupational therapist
- Support with the provision of equipment and medical supplies for the home
- The provision of oxygen concentrators and professional follow-up for eligible individuals who require continuous oxygen through the Home Oxygen Program
Self and Family Managed Care
Self-Managed Care enables clients with long term, chronic and stable care needs to accept full responsibility for their personal care as Self Managers. Individuals who choose this option rather than receiving their services through traditional Home Care Services are responsible for coordinating, managing, and directing the unregulated health care services they need to remain living at home and in the community
Family Managed Care enables clients with stable, chronic care needs to designate a family member to assume full responsibility for coordinating, managing, and directing the unregulated health services they need to remain at home.

Supportive Housing
Supportive housing is the right choice for people who require access to 24-hour supervision and some assistance managing with physical limitations, or ongoing health conditions such as dementia. Residents receive support and cueing with activities of daily living such as bathing, dressing, and medication reminders. Residents in supportive housing pay rent for their suite, and pay a service package fee which provides them with all their meals and weekly laundry and cleaning services.
