As a patient I have the right to

  • have my healthcare team deliver my care in a trustworthy and confidential manner;
  • have the relationship that I have with my healthcare team be built on trust, confidentiality and respect;
  • have access to appropriate healthcare services in a timely manner;
  • receive relevant, timely, appropriate care to give me the best health outcomes;
  • have my healthcare team share timely, correct information with me and all healthcare providers involved in my current care;
  • receive the best care, in the right place at the right time;
  • receive the best care in a safe place.

As a patient it is my responsibility to

  • share information with my healthcare team honestly, without fear and/or judgement;
  • actively participate in my care and work with my healthcare team;
  • participate with my healthcare team in decisions about my current and ongoing care;
  • work with my healthcare team to make the best healthcare plan for me;
  • ensure I understand what is happening so my healthcare team and I can make the best decisions for me.
