As a patient I have the right to
- have my healthcare team deliver my care in a trustworthy and confidential manner;
- have the relationship that I have with my healthcare team be built on trust, confidentiality and respect;
- have access to appropriate healthcare services in a timely manner;
- receive relevant, timely, appropriate care to give me the best health outcomes;
- have my healthcare team share timely, correct information with me and all healthcare providers involved in my current care;
- receive the best care, in the right place at the right time;
- receive the best care in a safe place.
As a patient it is my responsibility to
- share information with my healthcare team honestly, without fear and/or judgement;
- actively participate in my care and work with my healthcare team;
- participate with my healthcare team in decisions about my current and ongoing care;
- work with my healthcare team to make the best healthcare plan for me;
- ensure I understand what is happening so my healthcare team and I can make the best decisions for me.
